1. Personal Tax Return for the last three years for all principals
2. Business Tax Returns for the last three years or since the business started
3. Business Fiscal Year End Statement for the last three years
4. Current Interim Financial statement of the business
5. Occupational License
6. Special License to operate business (if applicable)
7. Bank and customer references
8. Resume for all principals
9. Supplier References
10. Business Plan
11. New and/or existing contracts
12. Application Fee of $100.00 (Non Refundable)
13. Copy of Drivers License & Social Security Card
